Steps to Follow
-Confirm the injury happened on a touch field- Note: The game official will write the injury on the game card at the time the incident happens.
- Read the Insurance information provided through teh link below.
- Down load and complete the incident form.
- Contact your local Affiliate and request a copy of the game card and ask the Affiliate to complete the Incident claim form (Affiliate Section)
- Lodge the incident form through the email address on the form.
